CAPS Transfer and Authority to Direct Form
The CAPS transfer and authority to direct form will enable you to use, in limited circumstances, a family member, carer or other responsible person to act on your behalf. Continence Aids Assistance Scheme (CAAS) clients can use their Centrelink Correspondence Nominee, Department of Veterans' Affairs (DVA) Trustee to act on their behalf to transfer to the CAPS. This form also enables you to direct your CAPS payments to an organisation that can assist you in the purchase of your continence products, if you wish.
The CAPS transfer and authority to direct form is available and can be downloaded (see below).
It is important that you include the reference number on the form when completing (the 7 digit reference number can be found on correspondence from Medicare Australia), and return it to Medicare Australia as soon as possible for processing. If the processing of your form is delayed, you may not receive the CAPS payment in the first two weeks of July 2010, but you will receive the full CAPS payment for 2010-11 of $497.79 (or two half payments if required) shortly after processing. For CAAS clients who have already registered their interest to receive this new form, a mail out will commence from 7 June 2010.

Only CAAS clients who have been invited to transfer by Medicare Australia may use the CAPS Transfer and Authority to Direct Payment form.
